Beyond Loyalty Program • $149/month or $1,600 annually (12-month commitment) • One complimentary facial every 30 days (Dermaplaning, Microdermabrasion, or Deep Pore Cleansing Facial) • 40 complimentary units of Dysport, Xeomin, or Jeuveau every 90 days • Unused units may roll over to the next quarter • Additional units: $10/unit • Brand-name Botox available at an additional cost • $100 off filler • 10% off full-priced services • 10% off skincare products

GENERAL TERMS

• Memberships are non-transferable and may not be shared. • Memberships automatically renew unless canceled in writing. • Automatic payment authorization is required. • Monthly dues are charged to the payment method on file. • No refunds will be issued for charged or prepaid membership fees. • Beyond the Surface may modify pricing, policies, and services with reasonable notice.

CANCELLATION

• Written notice must be provided at least 30 days before the next scheduled payment. • No refunds will be issued after payment has been processed.

MEMBERSHIP FREEZE

Eligible members may request one freeze per contract year for qualifying circumstances, including pregnancy, military leave, illness, or relocation.

Requirements: • Membership active for at least 30 days • At least one payment completed • Minimum freeze: 30 days • Maximum freeze: 3 months • Written request required

Benefits may not be used during a freeze period. Membership expiration dates will be extended by the freeze duration.

LIABILITY WAIVER

Members acknowledge the risks associated with treatments and agree to disclose any medical conditions that may affect services. Beyond the Surface providers do not diagnose medical conditions and are not liable for injuries, losses, or complications resulting from services or undisclosed conditions.

MEMBER ACKNOWLEDGMENT

By enrolling, the member confirms that they have read, understood, and agreed to all membership terms, benefits, limitations, payment obligations, cancellation policies, and liability waivers.

Members under 18 years of age must have a parent or legal guardian co-sign and attend appointments.

Pre-authorization for recurring credit card payments is required.
