Payment & Billing

Membership fees are charged automatically via Electronic Funds Transfer (EFT) or credit/debit card.
Monthly dues are charged on the 25th day of each month.
Memberships automatically renew unless cancelled in writing.
Members are responsible for maintaining valid payment information.
No refunds will be issued for charged or prepaid membership dues.

Cancellation

A written 30-day notice before the next scheduled payment is required.
Membership fees already charged are non-refundable.

Membership Freeze

Members may request one freeze per contract year for qualifying reasons (pregnancy, military leave, illness, relocation, etc.).

Requirements:

Active membership for at least 30 days
At least one payment made
Minimum freeze: 30 days
Maximum freeze: 3 months

Membership benefits cannot be used during a freeze period. Contract expiration will be extended by the freeze duration. Written request required.

Member Responsibilities

Members must disclose any medical conditions that may affect treatment.
Members must report any injuries or changes in health conditions.
Providers do not diagnose medical conditions.

Liability Waiver

Members assume responsibility for services received and release Beyond the Surface, its affiliates, agents, and employees from liability for injuries, losses, or damages arising from services rendered or undisclosed medical conditions.

Additional Terms

Memberships are non-transferable and may not be shared.
Beyond the Surface may modify pricing, policies, and regulations with reasonable notice.
Members under 18 require a parent or legal guardian to co-sign and attend appointments.
Pre-authorization for recurring credit card charges is required.

Auto-Renewal

Memberships automatically renew at the end of the annual term unless written cancellation is submitted. Monthly billing continues during renewal periods.

By enrolling, the member acknowledges that they have read, understood, and agree to all membership terms, conditions, benefits, limitations, payment obligations, cancellation policies, and liability waivers.
